Tackle Football Equipment Distribution Makeup Date is
Monday, August 19, 2019 from 11:00 AM-1:00 PM @ Location is at the Cube Smart Storage at 111 Danbury Rd, Wilton.
TIME SCHEDULE
Distribution arrival times are set by grade, please try to attend during your assigned time!
- 11:00 - 12:00pm grades 3-5
- 12:00 - 1:00 pm grades 6-8
PLEASE make every effort to attend the equipment pickup on 8/19. For those who do not pick up equipment on 8/19, you will be assessed a $25 special handling fee for equipment pickup later in the week.
In order for you to receive your equipment, please note that YOU MUST:
- Be Present: To ensure equipment is fitted properly and the for the safety of our players, equipment will not be distributed unless the player is present.
- Be registered: Completed all registration forms online.
- Be paid up: Registration fees must have been paid in full.
- Bring the following with you:
- Your completed Medical Form (ONLY the WYF Medical form is accepted)
- Your completed Parental Consent and Waiver of Liability Form
- Emergency Form for players with severe limitations (basic form is completed online)
- An equipment deposit check in the amount of $400, payable to "WYF", and DATED 12/1/19
